What is required at the time of application?

The application asks you to submit information about the organization undertaking or sponsoring the trip, the in-country host facility, a letter of invitation from a credible medical authority in the community where you intend on providing services, a complete needs-list for the trip, and a roster of volunteers traveling on the trip.
